The purpose of this study is to evaluate the impact of the Telehealth Advisor system on 1) adverse events related to heart failure; 2) patients' knowledge and self-management of heart failure; and 3) patients' quality of life. The Telehealth Advisor was developed by McKesson Provider Technologies, to be used in conjunction with traditional home health services. The Telehealth Advisor is FDA approved and is being used in home health agencies across the country. However, there has been no formal evaluation of this technology, with regards to its efficacy in health care. This project will evaluate the technology as part of routine home health care management.
