This study evaluates the outcomes of a team-based, patient centered, primary care program for persons with diabetes. The program was implemented in two Federally Qualified Health Centers operated by CareSouth Carolina. The clinics located in rural, federally designated Medically Underserved Communities, began implementation of the program in 1999. The study will be limited to the Medicaid population, and will assess changes in health care expenditures and key diabetes clinical indicators after implementation of the intervention. Medicaid data will be obtained from the South Carolina Office of Research and Statistics and clinical data will be retrieved from the clinics. Methods will generate statistical comparisons between the intervention group and control groups, with the goal of assessing the effectiveness of the CareSouth intervention.
