Student Request for Reference and Authorization

The Family Educational Rights and Privacy Act (FERPA) (20 U.S.C. § 1232g; 34 CFRPart 99) is a Federal law that protects the privacy of student education records and performance.
Students who request a faculty reference for use in applying for internship, employment, special programs or for other purposes must complete the Consent to Release Education Records Form indicating the request. The student indicates in writing on the form specific information that may or may not be included in the written reference. The student indicates in writing if the reference may be provided in writing or orally.
- Complete Student Consent To Release Education Records Form
- Submit signed Consent To Release Education Records Form to designated faculty member.
Faculty Member Responsibilities
- Complete reference within guidelines for request as outlined by the student.
- Submit completed Consent To Release Education Records Form to Undergraduate Coordinator for filing
